This issue is usually caused by a setting called SIP ALG. Please review common security blocks as there may be other factors.
Common Security Blocks
Step 1: Access your netGear Router
-The router is commonly accessed by typing in
http://www.routerlogin.net/
Step 2: On the left menu scroll down to WAN menu
Step 3: Switch the setting concerning SIP ALG (if checked, uncheck. If unchecked, check).
Step 4: Power cycle the netTALK product
Step 5: Test call in

Netgears can also be affected by firmware version installed and DNS.
